Item 5.02 Departure of Directors or Certain Officers; Election of Directors; Appointment of Certain Officers; Compensatory Arrangements of Certain Officers.
On April 12, 2015, Richard W. Turner, Ph.D., a member of CNS Response, Inc.'s Board of Directors (the "Board"), Audit Committee and Governance and Nominations Committee, passed away at the age of 68. Dr. Turner served as a member of the Board since February 25, 2013 and will be greatly missed.
